During our normal hours of operations, Monday - Friday, 9am - 5pm, you may call our office to submit your claims. Our office phone number is 574-875-1122. Use option #1 for claims assistance.

After hours or holidays, click on your insurance company's name to link you directly to their online claim page or call the claims hotline number provided.

*If you have submitted your claim directly to the insurance company, be sure to contact our agency at 574-875-1122 for us to provide proof of insurance to the state of Indiana so your license does not get suspended.
